NEW CLIENT (all new clients please fill this out)
If you are visiting for the first time please complete the Client Information Form. This form is for all new clients regardless of service scheduled.
If you are scheduled for a Massage and have not previously had a massage at YOUphoria please fill out the Massage Questionaire.
If you are scheduled for a Chemical Hair Service and have not previously had a Chemical Hair Service at YOUphoria please fill out the Chemical Serice Questionaire.
If you are scheduled for a Facial and have not previously had a facial at YOUphoria please fill out the Facial Questionaire.
If you are scheduled for a Body Waxing Treatment and have not previously had a body waxing treatment at YOUphoria please fill out the Waxing Release Form.
If you are scheduled for a Manicure or Pedicure and have not previously had a Manicure or Pedicure at YOUphoria please fill out the Mani / Pedi Form.
